Some calculations I just made. 4 round tournament 70 players average score 74. That gives 20,000 plus strokes played. About 8,000 would be putts.
That is a lot of opportunity to breach a Rule.

It doesn't happen much.

Putting greens are most likely. That is where there is a lot of handling the ball with Marking, lifting and replacing happening most putts. As well there is Marking to the side. That is a well known trap.
Putting greens get a lot of TV coverage too.

The problem with ball moved only detectable with slow motion video has been fixed with the new Decision.

It really shouldn't be too hard for players to do the right thing and do what is required by the Rules, on the green.
When it is your livelihood there is no excuse for not executing correctly.
