I've had two.

I had them on the same hole with the same clubs, about two months apart. :)

The first one wasn't in a comp.

But it counts to me. :)

My last game was at Pelican Waters, it cost me $80 plus there was a $1 Eagles nest. I paid for Dad too.
We played with Weets and Rascal. It was a shotgun start and we hit off on the 13th.
When we got to the 7th, I hit a really well struck drive and it went long, it did start a little right but held up fine. Weets hit a drive too, his went ok.
We get up to our balls and he was about 15m past me.
I was just over 120m out and because I was spewing that he was that far past me, I hit a sandwedge. Smashed it, it was all over the pin. It landed a couple of metres short and a touch left.
Weeti hits a great shot in too.
I missed my birdie, but had a tap in par. Weeti got his birdie.

Same, but one hio was just mucking around, even though I hit it from the tee, I hit out of turn in match play and was made to rehit

Minjee Lee was disqualified from the LPGA’s Kingsmill Championship for not signing her scorecard. It’s a mind-boggling error for a professional who is so accustomed to the post-round routine of checking and confirming scores.

Not that I know of, but I doubt the cards at most golf clubs with computerised systems are checked too closely. 

I have signed it after entering the score in to the computer before. 

I've also signed the wrong card when swapping in a 3.

the amount of times I am the one doing the scorecards and fudging my scores while I am drinking, signing the card or not is the least of the problems...
